Payment Terms

• Initial Payment Timeline: Patients must begin paying their visit balance within 60 days of the original billing.
• Pre-Collection Status: The visit balance will move to pre-collection status if no payment is received within 60 days.
• Final Notification and Grace Period: Our collection agency will send a final notification, initiating a 30-day grace period. During this time, payment can be made directly to PureView Health Center.
• Full Collections: If no payment or payment arrangements are made within the 30-day grace period, the visit balance will be sent to full collections.

• Last name
• Date of birth
• Phone number
• Statement code (located on your statement)

Financial Assistance

At PureView Health Center, no one will be denied care due to an inability to pay. We offer a sliding fee scale for patients experiencing economic hardship. If you are unable to pay your balance on time, please get in touch with our billing office at (406) 457-0000, option 4, to discuss payment arrangements.
