• PureView Health Center is a recognized medical home, meaning we provide the best evidence-based care that puts patients first and strives to continuously improve our approach in every way possible. 
• We use a PCMH approach to healthcare, which includes preventive care including routine check-ups, immunizations, integrated behavioral health, acute and chronic illness management, dental care and affordable prescriptions.
• We are committed to the PCMH model of primary care in order to increase our patients’ access to services that are offered outside the general medical care provided at our clinics.
• We deliver care in a PCMH model where you and your primary care provider, nurse, counselor and other specialists, as needed, work together with you to manage your health, injuries, and chronic conditions.
• The PCMH model allows you, as the patient, to be in charge of setting your own goals with the support of your health team. We work with patients to create better outcomes and save taxpayer money.
