Job Posting – Case Manager/Social Worker

Under the general supervision of the Behavioral Health Program Director, this position provides health education, health, social and environmental assessments and case management services to individual clients and families.

Duties and Responsibilities 

  • Provides case management services to individual clients and families
  • Performs health, social, and environmental screenings and assessments
  • Provides health education
  • Develops case management plans to address health, behavioral and other care related issues
  • Assists in community outreach
  • Provides care coordination and wrap around services for programs like Ryan White HIV/AIDS Program, Medication Assisted Treatment and Hepatitis C care
  • Assists in data entry, tracking and reporting of required program utilization
  • Provides client support, advocacy and linkage to community support services and organizations
  • Coordinates services with community service agencies
  • Monitors individual’s and families’ progress towards achieving plan goals
  • Maintains client records and documentation
  • Reports instances of abuse and neglect
  • Prepares reports on case management activities
  • Provides crisis assistance as necessary and resources permit
  • Participates, as needed, in the PureView Health Center’s (PVHC) performance improvement activitie\
  • Performs other duties as assigned

Required Qualifications – Experience & Education 

The above knowledge, skills, and abilities are typically acquired through a combination of education and experience equivalent to:

  • Four (4) year degree in Counseling, Social Work, or related field
  • One (1) year experience in case management
  • Health clinic experience preferred
  • Experience providing outreach to vulnerable populations preferred

Job Type: Full-time

Pay: $19.48 – $24.36 per hour
